🔴 Violations may include:

  • Unsafe or unsanitary conditions at a UPSHPA-accredited facility.
  • Unethical medical practices or patient mistreatment.
  • Failure to follow UPSHPA infection control & sterilization protocols.
  • Billing fraud or deceptive financial practices.
  • Practicing medicine without an active UPSHPA Accreditation.

📌 All reports are confidential and reviewed by the UPSHPA Compliance Team.


🔹 1. How to File a Complaint #

  • Step 1: Complete the online Report Accreditation Violations Form (below).
  • Step 2: Provide as much detail as possible, including dates, locations, and names of individuals involved.
  • Step 3: Attach any supporting documents or evidence (photos, receipts, medical records, etc.).
  • Step 4: Submit the form – a case number will be assigned for follow-up.
